As of October 1, 2020, Services Coordinators and Personal Agents who support people in in-home settings have the option of using the Oregon Needs Assessment (ONA) Risk Report to identify Serious Risk, as an alternative to the Risk Identification Tool (RIT).
This webinar gives the following guidance to SC/PAs working with people in in-home settings on how to:
- access and complete a person’s ONA Risk Report
- identify Serious Risk using the ONA Risk Report
- bring information from the ONA Risk Report into a person’s ISP Risk Management Plan
