Understanding different versions of ISP documents

From time to time, DHS may release updated versions of ISP forms to correct errors or to correspond with changes in policy or practice.

When a new version is released, it is identified by the date it was published. You can easily identify which version of a form you’re using by looking at the date in the lower right corner of the document.

When someone is new to services, create their first ISP using the most recent version available at OregonISP.org/forms.

When renewing a plan, you may continue to use the existing version of the plan as long as it remains on the list of currently accepted versions. Below is a list of currently accepted versions and a description of what has changed in each release.

Individual Support Plan

 5/2017 (PDF) Currently valid for use. “Print CDP” button does not work correctly. The on-screen definitions for “safeguarding interventions” and “safeguarding equipment” have been revised since this version.
 8/2017 (PDF)

Currently valid for use. “Print CDP” button does not work correctly. If the person makes a decision not to explore employment, be aware that the response to question 7 on the DNE will not print.

11/2017 (PDF) Currently valid for use.
7/2019 (PDF) Currently valid for use.
2/2021 (PDF) Currently valid for use. This release has fixed several technical issues and the file is smaller than previous versions. This release also includes translated versions of the ISP and an updated Large Print version.
