Person Centered Information lays the foundation of planning. It helps others to understand who the person is, what is important to them, and what they want their life to look like. This workshop is intended for providers who gather Person Centered Information, including provider organization staff and foster providers.
During this training we will:
• Explore the concept of Person Centered Information as a core component of planning with people
• Discuss the values behind understanding and recording a person’s perspective
• Provide tools and resources for better understanding a person’s perspective, a process often referred to as gathering Person Centered Information
