Desired Outcomes are the heart of an Individual Support Plan (ISP)—they capture how someone’s life will be different or better as a result of thoughtful, person-centered planning. Join us for this in-depth training to strengthen your skills and connect with colleagues. This two-day interactive online training is for CDDP Services Coordinators and Brokerage Personal Agents who develop Desired Outcomes.
Over the course of the training, participants will:
• Explore ideas for listening differently to better understand what’s Important TO the person and their vision for a good life
• Practice writing and refining Desired Outcomes with SCs and PAs across Oregon
• Discuss common challenges in creating and supporting outcomes, and work together to find strategies to address these challenges
