A Services Coordinator (SC) is an employee of a Community Developmental Disabilities Program (CDDP) who provides case management services to people of all ages. They use person-centered planning to assist people in planning, accessing, and monitoring the community-based supports and services that support their needs and goals. They also share resources and help families navigate service systems. This role is similar to that of a Personal Agent.
