A section within the ISP that addresses several areas of health and safety supports. This includes emergency preparedness, abuse prevention, what to do if the person cannot be reached, and back-up plan if the person’s primary support in unavailable. All known, serious risks in the person’s life that have been identified are listed in this section and information about how those risks are being addressed. Additionally, if the person has any safeguarding intervention or equipment in place, or an individually based limitation to an HCBS freedom, the ISP Risk Management Plan is the section in the ISP where this information is recorded.
