A Personal Agent (PA) is an employee of a Support Service Brokerage who provides case management services to adults. They use person-centered planning to assist individuals in planning, accessing, and monitoring the community-based supports and services that support their needs and goals. They also share resources and help families navigate service systems. This role is similar to that of a Services Coordinator.
