The Oregon Needs Assessment (ONA) is a tool which helps to assess a person’s functional needs (i.e. support needs) and helps to determine their level of care. The ONA is completed by a trained assessor, who asks a series of questions about a person’s abilities and support needs. Answers come from the person and others the person wants involved, such as their SC/PA, support providers, or friends and family.
