The Office of Developmental Disabilities Services IODDS) provides translated versions of key ISP forms. Updated translations of the ISP were released in July 2019 as part of the 7/2019 release.

Risk Identification Tool

Translation of this document is on hold. ODDS is developing a new needs assessment that will include risk indicators once it is complete, eventually replacing the Risk Identification Tool. Discussion is in progress about translation for the new Oregon Needs Assessment. Another update will be posted here when translation timelines are set.
