2/2021 Release of Updated ISP Form with Translations

Oregon’s Office of Developmental Disability Services (ODDS) has released an update to the ISP form (SDS 4118). This version has fixed several technical issues reported from the field. The file size is also smaller than previous versions. Here is a summary of the changes.

The 2/2021 update is considered optional, meaning renewing ISPs do not have to be transferred over to the new version. See the list of valid versions of the ISP form for more information.
